The National Consensus Project Quality in Palliative Care Award is presented annually to recognize organizations which have enhanced their palliative care services by implementing palliative care principles espoused by the National Consensus Project for Quality Palliative Care (NCP, http://www.nationalconsensusproject.org) and the National Quality Forum (NQF, http://www.qualityforum.org).

Up to eight NCP Quality in Palliative Care Award winners will be selected in Fall, 2008. The awards are supported by the Mayday Fund, based in New York City, in conjunction with the NCP.

Eligibility
All organizations or groups in the United States that provide palliative care are eligible for the award, including but not restricted to: hospices, hospitals, long-term care facilities, and other health care providers. 

Criteria
Programs will be evaluated on their ability to:

  • Show effectiveness in implementing the eight domains of palliative care described in the NCP’s Clinical Practice Guidelines for Quality Palliative Care and the 38 preferred palliative care practices defined in the NQF’s A National Framework and Preferred Practices for Palliative and Hospice Care Quality: A Consensus Report.
  • Quantitatively and/or qualitatively demonstrate significantly improved care through application of the Guidelines and Preferred Practices.

Applicants are asked to submit the following materials:

Supporting Materials
Applicants are encouraged to submit additional materials (e.g., strategic plans, informational brochures, letters of support, testimonials, annual reports, etc.) that help support the application and to demonstrate application of the NCP Guidelines in practice.

Submitting the Application
Applications are due by September 1, 2008. Applications should be mailed to: Dr. Ken Zuroski, National Consensus Project, One Penn Center West, Suite 229, Pittsburgh, PA 15276-0100. All application materials must be received at this address by 5:00 pm Eastern on the due date. Late and incomplete applications will be returned. Applications may be faxed to 412-787-9305. Award winners will be selected by October 1, 2008.
